Patients are advised to call their insurance prior to their appointment to confirm that their particular policy recognizes our office as participating, verify outpatient and office benefits and any authorization/referral requirements.

If you are self pay, payment in full is expected at time of service. We are able to offer a 30% prompt payment discount if payment is made at time of service.
